Current Champion
Guerrero Maya Jr. is the current champion, having defeated Rugido on June 2, 2023 in the finals of a tournament for the vacant title held at Arena México.

After a brief appearance in records from 1933, the championship was fully established by EMLL in a 1938 tournament, the surviving programs from which make for an incomprehensible format.[1] A tournament to crown the champion for a "newly created" Mexican Middleweight Title also turned up in 1940, albeit with an unknown outcome.[2]
